The approach to management of acute pulmonary embolism as a condition to be treated nonsurgically with systemic heparin anticoagulation was established more than 60 years ago. Patients with high-risk pulmonary embolism and cardiogenic shock appear to benefit from early thrombus removal, with rapid reductions in pulmonary obstruction and pulmonary vascular resistance from baseline and a lower risk of death than with anticoagulation alone, but at the cost of a nearly 10% risk of major bleeding.</subfield>
